Community Health Development Foundation

We are a non-profit organization with a mission to improve the health outcomes of underserved populations by expanding access to health and wellness related services and programming.

Our primary service is a resource connection program. Our resource navigator provides direct support and resource guidance to individuals in need of assistance with health and wellness access – from food to housing to legal counsel to mental health care. Our comprehensive resource database connects clients to local, state, and national funds, organizations, and programs addressing their needs, with wrap-around services that guide clients through every step of the process.

Health care and social service access can be confusing and complicated, and CHDF’s patient navigation program streamlines that process, raising awareness and engagement for those who struggle to meet the health and wellness needs of themselves and their families. We’re breaking down access barriers by connecting individuals to resources that already exist in their communities, and following up on these referrals to ensure that our clients receive the support they need.
