INFORMATION ON VETERAN'S BENEFITS, APPLYING FOR ARIZONA LONG TERM CARE, ALZHEIMER'S DISEASE AND RELATED DEMENTIA, COMMUNITY RESOURCES - ALL IN ONE LOCATION ON SATURDAY, NOVEMBER 6, 2010 FROM 8:30 AM UNTIL 1:30 PM AT GLENDALE ADULT DAY HEALTH CENTER, 6010 W. NORTHERN AVE., GLENDALE. FUNDED BY A GRANT FROM AREA AGENCY ON AGING IN PARTNERSHIP WITH THE FOUNDATION FOR SENIOR LIVING. THIS FREE EVENT INCLUDES A LIGHT BREAKFAST AND LUNCH. Glendale Adult Day Health Care Center provides daily specialized programs for  seniors and adults with disabilities. Specialized therapeutic recreational activities including group outings, choices of hot and cold nutritional meals and snacks, health care monitoring and medication administration by registered nurses are some of the services offered. Glendale Adult Day Health Care Center provides much-needed respite and resources for caregivers. Our program serves those in the Glendale and surrounding communities with diagnoses that include traumatic brain injury, developmental disabilities, mental health diagnoses, dementia, Alzheimer's disease, stroke and Parkinson's Disease. Available program funding sources include the Veteran's Administration, Arizona Long Term Care including Mercy Care, Evercare, Scan and Bridgeway funding sources, Division of Developmental Disabilities and Area Agency on Aging. There is also private pay, which is an hourly rate based on level of care. The Center is open Monday through Friday from 7:30 a.m. until 5:30 p.m. Please call 623-931-0983 to obtain additional information about our services.
