We provide the skills and expertise to help you manage your health care.

Each patient is assigned a Personal Health Manager who is either a Registered Nurse or Licensed Practical Nurse. The Personal Health Managers go with each patient to provider appointments. They help obtain copies of records and reports. They explain to the patient and family what the reports say and what are the next steps. They work with you to set up monitoring systems to be sure patients are complying with their provider's orders. They work with patients, providers, and the family.
