Summit Community Care Clinic is a Federally Qualified Health Center and the only safety net clinic in the Summit County and neighboring community area. We offer high quality primary care, oral health, behavioral health and reproductive health services to our patients, regardless of their ability to pay. We have been recognized locally, statewide and nationally for our integrated care model, and for the high quality of care that we provide.

Leadville School Based Health Centers
Our Mission: To Provide exceptional primary care, behavioral health and oral health services to Summit School District Students, staff and families in a safe and convenient location: their SCHOOLS!
Our SBHC site is located at Lake County High School and is staffed with Medical, Dental and Behavioral Health Providers. Lake County Intermediate School and West Park Elementary have Dental Health and Behavioral Health Providers and The Center has Dental providers frequent their locations.
